Waiver and Release of Liability
Although Holly Kidwell holds a clinical mental health license (LMHC, NM), soul care sessions are not therapy and are not intended to replace therapy or medical service.
I accept and acknowledge that the services provided to me by Adventures In Love LLC and Holly Kidwell are alternative modalities, not clinical mental health services, and nothing about my relationship with Adventures In Love LLC and Holly Kidwell shall create a patient/provider relationship. If I am in a crisis or emergency, I agree to call 911 or report to an emergency room or facility that provides emergency mental health services.
The United States of America currently has no licensing policy in regard to soul care, and Holly Kidwell is not a licensed Medical Doctor. Her purpose is to provide a safe space for clients to explore and experience healing while aligning their lives with their values.
Potential risks associated with soul care sessions include but are not limited to emotional distress due to the processing of events and the client’s perception of the nature of life. As mentioned above, soul care sessions will invite you to explore, and some things may come up that feel very uncomfortable. This discomfort is not a risk per se, but something that you should be aware of as a normal bi-product of the work. We will deal with discomfort as it arises, and you are ultimately in charge of the direction of the session. Potential benefits include a greater connection with yourself, your higher power, and your community; a greater awareness of purpose, strengths, and resiliency, and a greater sense of autonomy and peace. I agree to disclose any and all medical issues to Holly Kidwell.
Notwithstanding the risk stated above, I acknowledge I am voluntarily participating in soul care sessions and with knowledge of the risks involved. The undersigned hereby assumes all risk of injury or harm as a result of the activities specified above and agrees to release, indemnify, defend, and forever discharge Adventures In Love LLC and Holly Kidwell from all liability, claims, demands, damages, costs, expenses, and causes of action due to death, injury, loss, or damage to the undersigned.
